As a component of the Primary Care Intergration Program (ABHI) - a four-year program designed to strengthen the health system's focus on promoting good health and reducing the burden of chronic disease. Knox Division is working towards assisting GPs to work collaboratively with local service providers and private allied health providers. This section directs you to other sites where you can search for Allied Health Professionals and other government and non-government services and agencies in Victoria. This is to assist GPs in finding referral and local chronic disease care pathways.
