| The clinical education unit provides training, resources and support to GPs and Practice Nurses to assist them in improving and enhancing the management of their patients. |

| Provision of RACGP accredited clinical education session for GPs and Practice Nurses based on local needs and service shortage. |

| Currently under development is a 24-hour faxback service which give GPs access to evidence base clincial management guidelines and resources. |

| Patient Support Services |
|

Patient Tele-coaching – GPs will receive additional support in the clinical management of their patients with chronic conditions. Patients enrolled in this service will receive support and reminder calls to comply with clinical management guidelines as specified by their GP. |

Access to Allied Health Service – enables registered GPs to access psychologists for their patients with mental health disorders |

| Quality Use of Medicines |

Home Medicines Review - provides a Pharmacist Facilitator to work with GPs and Pharmacies to simplify the Home Medication Review process and increase skills in medication management. |

|
National Prescribing Service Program - offers one-on-one and group training with KDGP GP Facilitators who provides up to date information on drug therapies. It also provides access to case discussion groups and clinical audits, activities count towards your PIP QPI requirements. |
