Greater Eastern Primary Health in conjunction with Primary Mental Health Team has launched their electronic Mental Health Navigation Tool – www.gephmhtool.com .
It has been designed to assist Primary Care Providers to make decisions about which mental health services will be most beneficial to their patients/consumers.
This resource contains comprehensive information on GP Primary Health Services, Community Primary Mental Health Services, Private Mental Health Services and the Specialist Mental Health System within the Eastern Metropolitan Area
Included throughout the tool are referral forms and templates that can be downloaded for your use, together with links to websites for further information.

The General Practice Victoria (GPV) website contains templates for letters for GPs to use when responding to an AHP enquiry about a Team Care Arrangement (TCA)

This quick and easy survey (it should take about 5 mins to complete) contains a ‘menu’ of chronic disease management items and services for your Clinic to order from. Once we have your response we will put together a tailored resource kit and deliver it to your Clinic.

Mental Health Nurse Service - Free for your Patients
Knox Divisions’ MENTAL HEALTH NURSE SERVICE has expanded with the employment of three new nurses, bringing our team up to seven. These highly skilled Mental Health Nurses can support the management of your patients’ mental health needs. The nurses are available to see patients in your clinic, at Knox Community Health Service Wantirna site or at Knox Division’s Consulting Rooms at our new office in Scoresby.
