GP REFERRAL LINKS SERVICE
The Division provides a Referral Links Service which saves you time, by sourcing specialists and allied health services, informing you of the shortest wait lists, booking appointments and locating the service that is specific to you and your patient’s needs.
KNOX COMMUNITY HEALTH SERVICE GP LIAISON
The Division works closely with the Knox Community Health Centre to ensure new and existing services can be easily accessed by GPs. Faciltating communication between the GP and KCHS is also a priority of the program.
EASTERN HEALTH GP HOSPITAL LIASION SERVICE
The Primary Care Liaison team Facilitate communication between Hospitals and GPs concentrating on four action areas:

Notifications of admission , discharge , death , transfer & medication changes .

Referral letters to hospitals.

Education .

Provide process details to facilitate patient information exchange .
YOUTH HEALTH
The GP Speaker Service promotes harm minimization at local secondary colleges though its involvement in the Knox Safe Party Project.
